- Call sign and FCC facility ID
- Service type: FM, LPFM, translator, or other
- Community and state
- Existing and proposed coordinates
- Frequency or channel
- ERP, HAAT, antenna model, and antenna height
- The filing, change, study, or decision you need help with
- Any known deadline or filing window
